Eligibility Requirements

• You must be a resident of Madison County Virginia as shown by your driver’s license, utility bill, pay stub or other legal document.
• To receive medical care, you must be uninsured for medical expenses. This includes Medicare and Medicaid. You may, however, still be eligible for prescription assistance, and limited dental and vision care.
• Your total family income must be within our guidelines. You will need to provide documentation verifying all sources of household income.
• Most recent Federal tax return, if applicable.

Please note that you will be required to re-verify these eligibility requirements every 6 months.
