The Madison Free Clinic provides a wide range of in-house services, including primary care, blood work, and medication management. Additionally, we collaborate with local community businesses and organizations to ensure our patients have access to prescription services, mental health care, dental, and vision care. For specialty referrals, a UVA APPLICATION FOR FINANCIAL ASSISTANCE is available.

You may be eligible for medical care at the Madison Free Clinic if you:

• Are uninsured
• Are over the age of 18
• Live in Madison
• Have an annual income below 300% of the federal poverty level (please refer to the chart below)

We ask that you bring your photo ID, proof of income or a letter of support, proof of residency (such as a utility bill), and any relevant medical records or referral forms to your appointment at Madison Free Clinic. Renewal is every three to six months based on proof of income provided. 

Appointment scheduling is based on provider availability. Our providers currently see patients on Wednesday, Thursday, and Friday. Appointments are required. 

For prescription refill requests, please call Madison Drug Co. directly at (540) 948-4400 and provide your refill number. Your prescription will then be automatically forwarded to our providers for approval. There is a $3 co-payment. 

**Narcotic medications are not included in our coverage.
